Beebe Healthcare has opened a new location in an ongoing effort to provide follow-up care for patients being discharged from the Margaret H. Rollins Lewes Campus. Beebe’s Coordinated Care Center will bridge the gap between inpatient and crucial follow-up care for patients without a primary care provider. These transitional visits are vital to avoid rehospitalization and connect patients with a primary care provider for continued support. The office is located at 1305 Savannah Road in Lewes. Patients are given an appointment prior to their discharge. The Center does not accept walk-ins. “This is a great step forward in the value of care that we provide to Sussex County,” said Danielle Socrates, MBA, CPXP, Vice President of Value-Based Care Operations and Transformation. “This center will offer the right care for the patient at the right time, leading to lower costs for them by avoiding unnecessary emergency department visits and hospital readmissions. This center will provide value in our system, ensuring quality care at the right time and place.” The Coordinated Care Center is the latest evolution of Beebe’s strategic transformation to implement the best value-based-care practices in an equitable way for residents and visitors of Sussex County. It is important for healthcare systems to meet the needs of patients throughout the entirety of their healthcare journey. By looking at a patient’s continuum of care, the care team looks at the whole picture of a patient’s health. “This is what Beebe’s Care Coordination Program does for patients in Sussex County,” said Loretta Ostroski, MSN, RN, NE-BC, the Vice President of Integrative Care, Continuum of Care. “We help patients with chronic medical conditions navigate through the healthcare system, which plays a role in improving the health and well-being of individuals with chronic conditions. This step also contributes to the sustainability of the healthcare system by optimizing resource utilization and reducing costly complications and return trips to the emergency department.” This type of care coordination for patients is happening outside of the hospital and Coordinated Care Center. Beebe Medical Group physicians and advanced practice clinicians place an emphasis on integrated, preventative care, and Population Health outreach nurses bring resources to families with barriers to care in more remote areas. “Our goal is treating the patient rather than focusing on a specific disease or condition,” said Bill Chasanov, DO, MBA, FACP, Senior Vice President and Chief Health Systems Design Officer. “When we are able to partner with our patients, we can provide high quality, compassionate, and valuable care by developing individual plans that center around dignity and better outcomes.”
